What information do I need to submit my rebate?

To receive your rebate, you must first completely fill out the online submission form (available at www.cmspromocheck.com/pediasure/.)  Finalize your rebate submission by mailing in the following:
  1. Printout of completed online submission form.
  2. Your original, dated cash register receipt with the PediaSure purchase price circled. (Purchases must be made between 6/14/09 and 7/31/09)
  3. The original UPC symbol from the PediaSure product overwrap.

Mail in all of the above requirements to the address provided. Offer expires on July 31,2009, redemption requests must be received by August 31, 2009.

When will I receive my rebate check?

Please allow at least 6-8 weeks for delivery of your rebate check.

How do I check the status of my rebate?

To check the status online, visit http://www.cmspromocheck.com.  Please allow at least 4-6 weeks from postmark date for online rebate status information to appear.   This link will also allow you to send an email request for additional information, or you may call (877)322-8355 to speak with a representative.

Where will my check be mailed?

Your check will be mailed to the name and address provided on the form.

What will my check  look like?

Consumer will receive a full-size check folded into a self-mailer.
